Planned costs and what each service would include

• Psychiatric care: each evaluation and follow-up visit is planned to be charged separately. No subscription.
• Testosterone evaluation: the first assessment is planned to have its own fee. Initial diagnostic lab tests would be paid by you directly to the testing provider.
• Ongoing TRT (testosterone replacement therapy) membership: planned to include a defined set of clinical services, eligible medications and supplies, and medically necessary ongoing monitoring. Exact inclusions are still being verified.
• Not yet decided: amounts, exact inclusions, exclusions and payment terms.

Psychiatric evaluations and follow-up appointments will be charged separately. There will be no required psychiatric subscription and no requirement to join a TRT membership.

The fee schedule will state what each appointment includes and identify any separate costs for medications, testing, outside therapy or additional services.

The initial professional diagnostic evaluation will have its own fee. Initial diagnostic testing will be billed directly to you by the testing provider. We will explain how to obtain the applicable testing prices before you proceed.

An evaluation does not guarantee a diagnosis, prescription or acceptance into ongoing care.

The intended membership is inclusive of a defined set of clinical services, eligible medications and supplies, and necessary ongoing monitoring. The monthly price and exact included benefits are being finalized.

The final description will identify the eligible medication and supply options, included testing, fulfillment arrangements, communication limits and any costs payable to another provider. Psychiatric care will remain separate.
